Disk Read Errors?

anyone else get these here lately my ps2 has been giving me this alot, i read up on it and saw where it was better to turn the ps2 up, i done this and it help for a while now the dang this is doing it again. i swear to god this crap has got to stop, anyone got anything they have done to correct this?

I've gotten the same thing on XBOX. Cleaning the CD REALLY! well usually fixes it for me. I don't know if the problem is even caused by the same thing or not though.
